Hormone Therapy, Honestly
The short answer

For twenty years, women were frightened away from hormone therapy by a single study that has since been re-examined — and the fears, it turns out, were largely overblown. For many women, hormone therapy is one of the most effective things available for the symptoms of menopause and for protecting bone and joint health. It is not for everyone, and it is never a decision to make alone — but it deserves an honest, current conversation with a doctor who truly knows the field.

The study that scared everyone — and what we know now

In 2002, the Women's Health Initiative reported a rise in certain risks among women taking hormone therapy, and almost overnight, prescriptions collapsed. What got lost in the headlines: the study largely looked at older women, many years past menopause, and it was designed to test whether hormones could prevent chronic disease — not whether they safely treat the symptoms of the menopausal transition itself. A twenty-year follow-up and a 2024 re-analysis concluded that, for most women, the fears were overblown — and that estrogen alone carried an even more favourable profile than the estrogen-plus-progestin combination that drove the original alarm.

What estrogen actually protects

Estrogen is about far more than hot flashes. It helps protect the whole musculoskeletal system — so much so that researchers named the cluster of symptoms that follow its decline the musculoskeletal syndrome of menopause in 2024; more than seventy percent of women experience it. Hormone therapy has been shown to improve bone density and meaningfully reduce fracture risk, to ease the joint pain that so often arrives in these years, and — in a recent Duke study — to lower the risk of frozen shoulder. If you have wondered whether the aching hips, the stiff joints, or the stubborn shoulder are connected to your hormones, the science increasingly says: quite possibly, yes.

Bioidentical — and the fine print that matters

‘Bioidentical’ simply means hormones with the same molecular structure as the ones your body makes. Here is the distinction worth knowing: there are FDA-approved bioidentical hormones (such as estradiol and micronized progesterone), which are well-studied and regulated — and there are compounded bioidenticals, mixed to order by a pharmacy, which are not FDA-approved and can vary in dose and quality. Many specialists favour the approved, well-studied forms. It is exactly the kind of nuance a knowledgeable doctor will walk you through.

Timing — the ‘window of opportunity’

The evidence points to a window: the benefits of hormone therapy tend to be greatest, and the risks lowest, when it is begun during the menopausal transition or within roughly ten years of it, generally before age sixty. Started thoughtfully within that window, the protection for bone, heart and joints appears most real.

The non-negotiables

None of this is a reason to rush. If hormone therapy is something you are weighing, two things are essential. First, a physician who is genuinely current on the research — this field has moved faster than many practices have, and you want someone who has kept up. Second, regular bloodwork, roughly every six months, to keep your levels where they should be and adjust as your body changes. Done well, hormone therapy is monitored, personal, and ongoing — never set-and-forget.
